Under the supervision of the PACE Transportation/Facility Manager performs scheduling and dispatching activities in order to meet participant needs and ensure service delivery for PACE operations. Serves as the liaison between the drivers and the rest of the PACE IDT to ensure participant needs relating to transportation are identified planned for and met through excellent coordination of care and communication.

PACE Interdisciplinary Team Member
Represents transportation staff at interdisciplinary team meetings and care plan meetings bringing forward any PPT needs or safety concerns to the team as well as communicate any necessary PPT updates discussed by the IDT to the drivers.
Meets with participants to identify individual participant needs (ambulatory status assistive devises oxygen needs and cognitive impairments etc.) to ensure safe transportation and ensures information is communicated to drivers.
Documents direct PPT interaction and care planning activities in EPIC (Electronic Medical Record).
Interfaces with participants families contracted providers and the public in regard to issues related to the routine operation of transportation services.
Assists in identifying escort needs and work with PACE Home Care Coordinators as necessary to fulfill those needs.
Scheduling/Dispatching
Coordinates multiple schedules which require rigorous communication and collaboration with all departments. Reviews PACE clinic provider schedules outside appointment schedule therapy schedules recreation therapy schedule including field trips and special group meetings meal delivery schedule and supply delivery schedules to ensure PPTs are scheduled for transportation for all appointments/tasks.
Creates daily transportation schedules for all PACE vehicles in transportation software program (RouteMatch) and ensure that tasks are clearly defined and that the drivers have the information necessary to provide services in the safest manner possible.
Problem solves transportation scheduling conflicts short notice additions/changes to the schedule notifies appropriate staff of issues and provides possible solutions to meet participant needs.
Collaborates with discharge planners to ensure safe transportation needed at the time of hospital discharge.
Communicates with drivers when PPTs are ready for pick up from contracted provider offices and other changes to their schedules in the safest manner possible.
Maintains and updates RouteMatch database with any new PPTs or providers that need to be added and addresses or contact information that needs to be updated.
Performs daily verification for all vehicles to ensure times and odometer readings are within appropriate ranges.
Late afternoon runs daily trips report exports it and then uploads schedules as PDF onto PACE Sharepoint so that the rest of the PACE staff can access next day schedules.
Works with RouteMatch tech support to problem solve program and driver tablet issues.
Administrative
Assumes a leadership role in the development implementation and refinement of transportation services
Assists Transportation/Facility Manager with review of program utilization data quarterly indicators financial performance participant/caregiver satisfaction surveys and staff competency. In conjunction with Transportation/Facility Manager identifies areas for performance improvement.
Driving
Provides transportation services to program participants to and from the PACE Center and/or medical appointments according to assigned route/appointment schedule.
Adheres to all agency department and State of Iowa Department of Transportation rules and regulations related to providing safe and comfortable transportation services to participants. This includes but is not limited to use of gait belts proper loading and unloading of passengers proper wheelchair securement and safe and lawful driving.
Reports any vehicle safety/maintenance issues to the Transportation Manager or designee as they arise.
Education:
High School diploma or equivalent required.
License(s)/Certification(s):
Maintains valid drivers license (Chauffers endorsement for Iowa license) or other state equivalent license
Maintains valid proof of Auto insurance
Maintains good driving record
Maintains current CPR certification
Maintains Mandatory Adult Abuse Reporter Certification
