Summary:
Provides patient care in a Hospice setting according to established standards of care in collaboration with RN Care Managers and Medical Providers as members of the primary care team. May work at inpatient or outpatient setting.

Qualifications
Other information:
*High School Diploma or GED
*Practical Nursing Formal Education approved by the NM Board of Nursing
*Licensed Practical Nurse-NM
*BLS required
*PDS IV Certification course (within orientation period)
*Valid NM Drivers license; meets qualifications of PHS drivers policy.
