Job Description

Job Purpose

The Fatherhood & RISE Coordinator II provides case management outreach and educational engagement to:

  • Encourage fathers to participate actively in their childrens lives especially in school-based activities.
  • Support re-entering individuals by linking them to services that reduce barriers and prevent recidivism.

The ideal candidate is a relationship builder with experience supporting families and individuals impacted by the justice system possessing strong organizational skills and a commitment to equitable service delivery.


Fatherhood Program Responsibilities

  • Deliver groups and individual support services to help fathers improve parenting techniques and engagement with their children.
  • Assist fathers in addressing co-parenting challenges and accessing wraparound services.
  • Collaborate with schools to plan and host events that promote healthy father-child family systems relationships.
  • Conduct targeted outreach and recruitment efforts to engage low-income and underrepresented fathers.

RISE Re-entry Program Responsibilities

  • Accept referrals and conduct needs assessments and intakes for formerly incarcerated individuals.
  • Develop and monitor individualized re-entry and personal action plans.
  • Provide case management and wraparound support.
  • Serve as a liaison with parole officers transitional housing employers and community partners.
  • Facilitate support groups and educational classes both in-person and virtual.

Other Functions: Maintain accurate and timely documentation including case notes group rosters assessments evaluations and pre/post-tests. Input data into program databases. Attend agency community and funder meetings and contribute to organizational decision-making. Participate in all required training supervision and professional development. Work a flexible schedule that may include evenings or weekends. Works effectively and positively with all co-workers volunteers and participates in professional or community networks as appropriate. Other duties as assigned by supervisor.

Qualifications:

:

Associate degree or some college in Social Work Human Services or related field.Minimum 2 years of experience in case management or working with re-entry populations or family systems. Knowledge of wraparound service delivery. Valid Ohio Drivers License operational vehicle and insurance. Ability to pass BCI/FBI background check and drug screening. Strong Microsoft Office skills and comfort with database entry.


Preferred:

Bachelors degree in social work Human Services or related field. 3 years of case management experience. Experience in mentoring group facilitation and working with fathers and family systems. LSW LPC or license-eligible credentials. Familiarity with the Standards of Quality for Family Strengthening & Support. Must have and maintain operational vehicle and insurance. Must be able to pass a BCI and FBI background check and drug pre-screening.

SUPERVISOR: Associate Director HOURS WORKED: Full-time/Exempt (40 Hours)

SALARY: $45000.00
