  • Salary: 33366 - 39513 per annum (pro rata for part time)
  • Hours: 37 hours week - Flexible working options available

Your contribution will help ensure they discharge at the right time to the right location alongside continually developing and building upon the trusted assessment processes between hospital teams and care providers to improve the process for everyone and reducing hospital delays. Your role and responsibilities

  • Building supportive relationships with the care market
  • Monitoring quality and performance of services and challenge poor practice
  • Being proactive and drive opportunities to improve systems and processes
  • Advocating and negotiating for the best outcomes for people care homes and the health and social care system
  • Leading on the continuous development of a trusted assessment process to prevent discharge delays from hospitals
  • Researching learning from implementing and reflecting upon best practice
  • Providing insight and feedback on how to develop and change current systems and lead on change where appropriate.

As you can see this is an exciting opportunity to support people who have been in hospital and are now ready to discharge to transfer safely with care and compassion to their onward pathway be this returning home or moving into a care setting such as a care home.

Our ideal candidate will have:

  • Excellent organisational skills time management and flexible approach to working.
  • An ability to communicate with customers and colleagues across health and social care in a tactful manner show empathy and resolve conflict effectively
  • An ability to influence the actions of others and negotiate solutions which are acceptable to all concerned
  • An ability to express ideas in a clear concise and tactful manner
  • A creative approach to care solutions and will consider alternatives alongside traditional care services such as the voluntary community faith and social enterprise sector providing people with choice and control over their care options.
  • Have the ability to demonstrate Suffolks ASPIRE values and behaviours in your everyday work
  • Be open to change and positively adjust behaviours to suit new procedures systems or responsibilities.

The team

Within Adult Social Care the Strategic Commissioning and Contract Management teams (SCCM) work to realise ASCs strategic priorities with its suppliers and delivery partners. This includes shaping the market and services to maximise the range of quality services promote peoples independence whilst achieving long term financial sustainability. And most importantly promoting wellbeing taking this into account in all our decision-making.

Your will work closely with critical partners such as our Brokerage team Social Work services Health colleagues including hospital discharge and clinical staff Residential and Nursing care providers Home Care providers including Home First and most importantly people and/or their advocates or next of kin.

The Waveney SCCM team is a very supportive team. We help one another continually support with individual growth and professional development and aspire to maximise on team members skills and talents providing them with a portfolio that allows each to shine and demonstrate their value to the organisation.

Whilst our office day is usually a Tuesday members of the team who work to a hybrid model of office site visits and home working are in the office at least two days a week and all are contactable via our MS Teams site so you are never alone.
