Job Description

STAT Home Health is hiring an Experienced Home Health OASIS Processor (RN Preferred)

Location: Remote
Job Type: Full-Time
Experience Required: Minimum 3 years in Home Health Case Management
Software: Mandatory HCHB (Homecare Homebase) experience
Certification: OASIS Certification preferred (COS-C or HCS-O)

Job Summary:

We are seeking a detail-oriented and experienced Home Health OASIS Processor to join our dynamic post-acute care team. The ideal candidate is proficient in OASIS data collection and processing has a strong understanding of Medicare Conditions of Participation and excels in the Homecare Homebase (HCHB) platform. This role plays a critical part in ensuring accurate documentation and compliance with all regulatory standards contributing to exceptional patient care and optimized agency performance.

Key Responsibilities:

  • Review audit and process OASIS documentation for accuracy compliance and completeness
  • Ensure all OASIS data sets are completed and locked timely per CMS and agency standards
  • Collaborate with clinical staff to clarify documentation discrepancies or missing data
  • Verify appropriate ICD-10 coding and HHRG scoring in alignment with patient acuity and plan of care
  • Utilize Homecare Homebase (HCHB) for workflow processing QA and documentation tracking
  • Provide clinical and technical support to field clinicians for OASIS and HCHB-related questions
  • Monitor and reconcile outstanding documentation to maintain timely billing cycles
  • Participate in QAPI meetings and contribute to quality and compliance initiatives
  • Support the case management team with coordination of care and interdisciplinary communication
  • Maintain up-to-date knowledge of Medicare regulations PDGM and HHVBP requirements

Qualifications:

  • Minimum 3 years of experience in home health case management or clinical documentation
  • Mandatory proficiency with HCHB (Homecare Homebase)
  • Strong working knowledge of OASIS-E PDGM guidelines and CMS home health regulations
  • OASIS certification (COS-C or HCS-O) preferred
  • Active compact RN License.
  • Detail-oriented organized and self-motivated
  • Ability to work independently and meet deadlines in a fast-paced environment
