The Supportive Employment Specialist reports to the Community Support Services (CSS) Program Manager in coordination with the Certified Community Behavioral Health Clinic (CCBHC). They work in collaboration as part of the CCBHC Grant Team. The Supportive Employment Specialist addresses employment needs by completing a Vocational Profile and Individual Employment Plan with all clients in need of Supportive Employment Services. They provide care coordination in collaboration with the care team to youth and/or adults with mental health and/or substance use disorders including those with serious and persistent mental illness and chronic addiction. They coordinate with external employment referral sources such as Vocational Rehabilitation Work One and community employers. They address CCBHC vocational rehabilitation and psychiatric rehabilitation criteria identify gaps in service areas and coordinate with the CCBHC Grant Team for data collection. They will assist with enhancing services in support of the CSS treatment team. The Supportive Employment Specialist will develop existing internal and external partnerships complete project presentations and seek to increase understanding of CCBHC goals to improve outcomes related to employment needs of CCBHC clients.


EDUCATION


4 year / Bachelors Degree (Required)


LICENSE & CERTIFICATION REQUIREMENTS


Required Certifications/Licensures: (Current Indiana Drivers License with valid vehicle insurance)


EXPERIENCE


2 years: Two (2) years of social service experience in a volunteer or paid role. (Preferred)


ESSENTIAL FUNCTIONS (JOB DUTIES)


Patients First: The Supportive Employment Specialist collaborates as part of a multidisciplinary care team by supporting the comprehensive psycho-social assessments. They help clients address vocational rehabilitation needs can deescalate clients displaying aggressive behaviors and document services provided in Electronic Medical Record (EMR) based on policies and procedures. When necessary will transport clients in personal vehicles.
Relationships: Applicants for this position should be able to collaborate with others in a team setting have excellent communication skills and a positive attitude toward problem-solving. They work as part of the Certified Behavioral Health Clinic (CCBHC) and in coordination with Community Support Services (CSS). They assist the CCBHC Grant Team with developing and enhancing internal Page 2 of 3 and external partnerships.
