Job Title: Participant Escort
Department: Clinical Operations
Location: 490 East Ridge Road Rochester N.Y.
Hours Per Week: 20 hrs
Schedule: Monday thru Friday 4 hours shift

SUMMARY: The Participant Escort will meet and assist ElderONE participants at their scheduled appointments.

RESPONSIBILITIES:

• Complete activities as listed within the participant care plan as required during each encounter

• Meets participants at scheduled appointments

• Assists participants with checking in and checking out for appointments

• Provides assistance with needs while attending the appointment including (but not limited to): supervision transfers dressing and undressing toileting assist with positioning

• Communicates with Appointment Coordination team and Transportation to review any cancellations no shows change in appointment times

REQUIRED QUALIFICATIONS:

• Reliable transportation to meet participants at scheduled appointment

• Experience with elderly population preferred

• Experience with direct patient care preferred

• HHA CNA PCA preferred

PAY RANGE:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
