Job Description
Job Title: Team Case Assistant Home Care
Department: ElderONE
Location: 490 E. Ridge Rd. Rochester NY 14617
Hours Per Week: PART TIME- 20 Hours/Week
Schedule:Day/Evening
RESPONSIBILITIES:
- Responsible for coordinating participant services and staff scheduling. Assures complete and accurate schedules for ElderONE staff primarily HHAs and Nurses.
- Creates and schedules case assignments and assists with timekeeping as needed. Provides a range of supportive services to Home Health Aide operations of ElderONE.
- Assures appropriate routing of calls and assistance with transitioning participants through care.
- Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers needs are met.
- Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee.
- Checks email in a timely manner providing follow up to assure senders needs are attended appropriately.
- Collects organizes and reviews various programs for information and reporting and initiates follow up.
- Updates the EMR per agency standards.
- Performs functions related to transfers or discharges as required by regulation.
- Builds and maintains strong communicative relationships with the team.
- Provides exemplary customer service.
- Performs general office duties/projects including but not limited answering phones documentation/order processing copying filing mailing and obtaining/ordering office supplies.
- Assists in orientation of new staff role modeling participates in quality assurance activities.
- Attends team meetings and other internal meetings as requested.
PREFERRED QUALIFICATIONS:
- Excellent interpersonal skills and customer service skills.
- Ability to multi-task in a fast-paced environment
- Proficient use of Microsoft office.
- One year of experience in home care setting or medical office preferred.

S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. PAY RANGE:
$17.75 - $19.65CITY:

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience relevant qualifications specialty internal equity location and contracts.
