About the Role

We are looking for a Community Support Worker to join our Individualised Community Living Strategy (ICLS) program providing outreach support to clients accessing the servicebetween the Mandurah to Morley areas. The service is for clients with a mental health diagnosis who are eligible for a support package. The role of the Community Support Worker is to help the client access their local services and maximise their recovery success.

Key Responsibilities

  • Provide dedicated support to clients with a mental health diagnosis focusing on recovery and community living goals
  • Coach and encourage independence through daily routines and decision-making
  • Assist with cooking cleaning and home upkeep empowering the client to take control of their daily living
  • Encourage community access and engagement in local activities
  • Support capacity building and self-management of lifestyle and wellbeing
  • Work collaboratively with clinical and mental health support teams
  • Follow and contribute to individualised support plans
  • Thursday and Friday daytime availability REQUIRED
  • Flexibility to travel up to 40 minutes from home as required. Travel will be within a 40 km radius of the Perth metro area

Skills & Experience

  • Certificate IV in Community Services or equivalent
  • Demonstrated experience working autonomously with people with mental illness or psycho-social disability
  • Passionate about client outcomes and mental health advocacy
  • High computer literacy and administration skills
  • An unrestricted Western Australian drivers licence and access to a reliable vehicle
  • A current First Aid Certificate which includes the course HLTAID003 or HLTAID011

Successful candidates will be required to clear all state-based probity checks; including but not limited to a National Criminal History Record Check and NDIS Worker Screening Check.
