Job Description

The Hospice Social Worker identifies social and emotional problems impacting the patients medical condition and helps the patient to establish goals along with developing a social work plan of care. Knowledge and use of community resources is essential in addition to close collaboration with members of the hospice team and coordination of care with community agencies and other health care providers. Hospice social work documentation of evaluation visits and progress notes are completed as part of the patients Medical Record. The hospice social worker participates in discharge planning interdisciplinary conferences in-service programs and acts as a resource for other hospice team members.

Requirements:

  • Bachelors degree in social work psychology sociology or other related field required
    • Masters degree in Social Work preferred
  • At least one (1) year of social work experience in a health care setting required
    • Previous home care experience preferred.
  • Competence to deal with clients of varying ages and stages of development is required.
