Job Description
This Case Manager position requires a Registered Nurse to provide skilled nursing care case management and potentially hospice care to patients facing age-related frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Rotating weekend coverage is expected.
- Medication administration and treatment planning.

- Bachelors degree in Nursing or related field.

- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and personal vehicle for field work (if applicable).
- Position may involve field work requiring a personal vehicle.
- The role may involve providing hospice care depending on the specific needs of the patients.
