PRIMARY FUNCTION
The Pediatric Behavioral Health Case Manager supports the care coordination needs of pediatric patients and families affected by mental health or substance use disorder. Works with patients and their families to develop and implement a person-centered care coordination plan ensuring access to the supports and resources needed to improve mental health and health outcomes. Collaborates closely with other members of the WHC care team as well as schools and other service organizations.
PRINCIPLE DUTIES
  1. Assess patient and family needs and develop patient centered care coordination plan that addresses patient/familys social barriers to improving mental health and health outcomes. Provide close follow up and care coordination to ensure patient and family can access needed resources.
  2. Collaborate with other WHC care team members school or daycare personnel and other service providers involved with the patient.
  3. Provide health education to patients and families to reduce the impact of mental health condition on health outcomes.
  4. Serves as an internal reference for resources available to support pediatric mental wellness.
JOB REQUIREMENTS:
Minimum Education & Experience: High School Diploma or GED. Bachelor Degree in Social Work least two years experience in a patient care setting. Experience in case management and/or working with children and families preferred.

Qualifications:
Successful candidates will be skilled in identifying social barriers to mental health services for children and families enthusiastically engage with patients and be adept in quickly developing working relationships patient action plans and follow-ups that support progress toward patient health goals. Candidates must be proficient in time management able to multi-task competing priorities and be critical thinkers. Ideal candidates will have strong verbal and written communication skills timely documentation and maintain a high level of professionalism.
TYPICAL WORKING CONDITIONS
Office environment with frequent contact with staff and the public. Work may include transporting patients/caregiver to appointments when needed in company vehicle.
ACCOUNTABILITY
Reports to Patient Support Services Manager.
