POSITION SUMMARY

Consistent with the interprofessional setting and the mission vision and strategic directions of CHEO the Social Worker provides psychosocial assessments and therapeutic intervention to children and youth who present with mental health challenges primarily in our Community Based Mental Health Program. The Social Worker will work with individuals and families providing clinical interventions and consultative services within the CAPA (Choice and Partnership Approach) framework to care which includes the Rapid Response pathway. The Social Worker will work with children and youth up to 18 years old and their families providing evidence-based individual family and group psychotherapy and interventions crisis intervention and risk assessments.

The Social Worker will have an active role in program development service evaluation parent and staff education and skills-based training as required. The Social Worker will participate in providing mentoring and training opportunities to Social Work students studying at the BSW/MSW level.

The Social Worker will be a member of the CHEO Discipline of Social Work and will participate in the Social Work Coverage Model.

PRIMARY DUTIES

  • Conducts comprehensive (psycho-social and family) assessments by methods such as interviewing the family obtaining relevant information gathering social data regarding the child/youth and family and formulation of assessment and plan of intervention in accordance with professional practice standards and clinical policies.

  • Provides counseling and support to families by offering brief individual and family counseling crisis intervention risk assessments and conflict resolution related to treatment decisions child welfare mental health and comorbid conditions.

  • Provides therapeutic interventions using various modalities including Cognitive Behavioral Therapy and Trauma-Informed approaches.

  • Provides family therapy and group-based interventions.

  • Engages in training and the delivery of education to Social Work students and health care professionals.

  • Advocates on behalf of families with respect to services and support and coordination of community resources.

  • Maintains current knowledge on social work evidence-based practice by such methods as reviewing literature consulting with clinical and community experts in the field evaluating clinical practices and participating in professional development activities in order to improve care through new practices.

  • Performs administrative tasks such as maintaining clinical documentation progress notes workload statistics and updating health records in accordance with established standards.

  • Participates in interdisciplinary team meetings.

  • Liaises with various community partners including the School Boards child welfare organizations and community mental health providers.

  • Participates in and supports clinical research activities with the inter-professional team members.

  • Participates in committees within the discipline of Social Work

  • Carries other responsibilities as assigned.

QUALIFICATIONS

  • Vulnerable Sector Check (Essential)

  • M.S.W. graduate from an accredited university program in Social Work (Essential)

  • Member of the Ontario College of Social Workers and Social Service Workers and broad knowledge of Social Work theory practice and regulations established by the College (Essential)

  • Demonstrated expertise and knowledge about children presenting with acute and complex mental health Issues (Essential)

  • Demonstrated ability to integrate the following approaches into practice: Strength-Based Perspective Family Systems Grief & Bereavement Crisis Intervention and Brief Solution-Focused therapy (Essential)

  • Demonstrated clinical expertise as a family therapist (Essential)

  • Clinical experience and knowledge in delivery of group-based interventions (Essential)

  • Experience using CBT and Trauma-Informed approaches to care (Essential)

  • Experience and skill in teaching family therapy (Preferred)

  • Demonstrated ability to deliver educational seminars and teaching to Social Workers and health care professionals (Preferred)

  • Formal training in family therapy (Preferred)

  • Minimum of 5 years experience in children/youth mental health within community-based or pediatric settings (Essential)

  • Fluency in both English and French (Preferred)

  • Demonstrated ability to work independently (Essential)

  • Demonstrated ability to conduct and document a comprehensive psychosocial assessment (Essential)

  • Demonstrated ability to plan organize and prioritize work (Essential)

  • Demonstrated ability to work collaboratively and effectively within an inter-professional environment with members of the health care team clients and their families and external agencies (Essential)

  • Knowledge of community resources (Preferred)

  • Cultural competencies in order to work efficiently in cross-cultural situations (Essential)

  • Ability to operate a computerized patient care information system (Preferred)

RSUM DES FONCTIONS

Conformment au milieu interprofessionnel ainsi qu la mission la vision et aux orientations stratgiques du CHEO le ou la titulaire mne des valuations psychosociales et familiales et offre une intervention thrapeutique aux enfants et adolescents aux prises avec des dfis en sant mentale essentiellement au sein de notre Programme de sant mentale en milieu communautaire. Il ou elle fournira des interventions cliniques et des services de consultation aux individus et aux familles sur la base du cadre de travail intitul CAPA (Choice and Partnership Approach). Il ou elle travaillera avec des familles des enfants et des jeunes jusqu lge de dix-huit ans et leur offrira des interventions et de la psychothrapie fondes sur des donnes probantes sur une base individuelle familiale ou au sein dun groupe.

Le ou la titulaire jouera un rle actif dans llaboration du programme lvaluation du service le dveloppement professionnel et la formation base sur les comptences tel que requis. Il ou elle participera aux occasions de mentorat et de formation auprs des tudiants en travail social qui prparent un baccalaurat ou une matrise en travail social.

Le ou la titulaire sera membre du groupe professionnel des travailleurs sociaux et travailleuses sociales et participera au modle de remplacement du travail social au CHEO.
