Job Overview

Title: Physical Therapist Home Health
Department: Therapy / Rehabilitation
Type: Travel / Contract (13 Weeks)
Schedule: Day Shift 5x8-Hour (08:00 AM 4:30 PM) No weekends no on-call
Start Date: 08/18/2025

Job Description

Summary:
We are seeking a skilled and experienced Home Health Physical Therapist (PT) for a 13-week assignment covering Medford Oregon and nearby areas. The ideal candidate will provide 1:1 patient care averaging up to 6 visits per day with a standard of 30 productivity points per week. This position does not accommodate first-time travelers or alternative schedules such as 4x10s or 3x12s.

Responsibilities:
  • Deliver skilled home health PT services to a diverse patient population

  • Conduct assessments and implement treatment plans according to patient needs

  • Complete accurate and timely OASIS documentation (required)

  • Provide patient and family education on home exercise programs and recovery goals

  • Travel to patient homes using own reliable transportation within a defined service area

  • Collaborate with interdisciplinary healthcare teams for optimal patient outcomes

  • Maintain compliance with all clinical and documentation standards

Key Requirements:
  • Minimum 1 year of recent skilled home health PT experience

  • OASIS experience required must be clearly disclosed if not met

  • BCLS certification required

  • State PT license required (pending accepted if active by start date)

  • Must have own transportation

  • No weekend on-call or block scheduling available

  • Locals within 50-75 miles accepted at same rate

  • First-time travelers not accepted

  • RTO must be approved by the hiring manager

  • Guaranteed hours apply: facility may cancel up to one shift per pay period (biweekly)

Qualifications

Education:

  • Graduate of an accredited Physical Therapy program

Experience:

  • At least 1 year of recent skilled home health experience required

  • Experience working with a variety of diagnoses including orthopedic neurologic and post-surgical rehab

License:

  • Active Oregon PT license (or pending with confirmation of issuance before start)

  • BLS Certification (American Heart Association or equivalent)

Skills:

  • Strong documentation proficiency especially with OASIS

  • Ability to manage caseload and productivity independently

  • Excellent communication and interpersonal skills

  • Experience working with patients with the following conditions:

    • Orthopedics: Cervicalgia post-op spinal surgery shoulder repairs THR TKR low back pain

    • Neurologic: Parkinsons MS spinal cord injury stroke recovery cognitive impairments

  • Settings experience (preferred): Acute Care SNF Outpatient Clinic Rehab Hospital

  • Additional skills: Patient/family education body mechanics assessment home exercise planning
