Position Title:

Managed Care Coordinator (LBSW LSW LMSW LCSW LPC)

Job Description:

LOCATION: This is a virtual eligible position for the Tidewater area of Virginia. Residency in the Hampton/Newport News area is preferred. Virginia residency is required.

HOURS: General business hours Monday through Friday.

TRAVEL: Some travel within the Tidewater region may be required.

This role enables associates to work virtually full-time with the exception of required in-person training sessions providing maximum flexibility and autonomy. This approach promotes productivity supports work-life integration and ensures essential face-to-face onboarding and skill development.

The Managed Care Coordinator Clinician Non-RN is responsible for contributing to the care coordination process by performing activities within the scope of licensure. This includes tasks such as conducting telephonic or face-to-face assessments for the identification evaluation coordination and management of members needs including physical health behavioral health social services and community services and supports. This position operates with overall responsibility for the members case as required by applicable state law and contract.

Primary responsibilities include but are not limited to:

  • Identifies members for high risk complications.

  • Obtains clinical data.

  • Identifies members that would benefit from an alternative level of care or other waiver programs.

  • Conducts additional assessments as necessary and develops monitors evaluates and revises the members care plan to meet the members needs.

  • Participates in coordinating care for members with chronic illnesses co-morbidities and/or disabilities and ensures cost effective and efficient utilization of health benefits.

  • Decision making skills will be based upon the current needs of the member and require an understanding of disease processes and terminology and the application of clinical guidelines but do not require nursing judgment.

Required Qualifications

  • Current unrestricted LSW LCSW LMSW LBSW or any clinical license other than an RN from the Commonwealth of Virginia is required.

  • A minimum of 2 years of experience in working with individuals with chronic illnesses co-morbidities and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience which would provide an equivalent background.

  • May require state-specified certification based on state law and/or contract.

Preferred Qualifications

  • I/DD and/or behavioral health experience is very strongly preferred.

  • Care Coordination experience is preferred.
