Matrix Providers is hiring a Nurse Practitioner Family Practice to join our team of talented professionals providing healthcare services to Military Service Members and their families at Naval Medical Center Portsmouth VA.

Job Summary:
Analyze the results of the health care plan modify the health care plan as needed and implement and participate in follow-up.
Participate in meetings to review and evaluate the care provided to patients identify opportunities to improve the care delivered and recommend corrective action when problems exist. Should a meeting occur outside of the HCWs regular working hours the HCW shall be required to read and initial the minutes of the meeting.
Participate in the provision of monthly in-service training to non-health care-practitioner members of the clinical and administrative staff on subjects germane to the HCWs specialty.
Provide for an optimum physical environment for patients and staff emphasizing good safety practices and cleanliness.
Support preventive maintenance and report equipment failures and inadequacies.
Promote economic utilization of equipment supplies and be cost conscious when ordering lab and radiological studies.
Know the location and operation of emergency equipment.
Adhere to departmental and hospital safety guidelines.
Diagnose patients with common acute conditions illnesses or minor trauma within legally accepted protocols or in accordance with subsequent instructions.
Obtain patient health and developmental history.
Perform and record a health appraisal including physical assessment and evaluation.
Request diagnostic testing including but not limited to x-rays and laboratory tests as deemed necessary.
Formulate a health care plan for clinic patients emphasizing self-care responsibility through the participation of the patient family physician and other health care professionals.
Treat patients with common acute conditions illnesses or minor trauma within accepted protocols in collaboration with MHP team members (other providers Nurses Pharmacists etc.) and specialists as clinically indicated.
Engage in proactive population-based health management providing regular surveillance to patients need for preventive services and management of chronic disease leveraging information technology decision support tools and the members of the medical home port team
Identify resources and coordinate referrals for patients and families requiring further evaluation and services.
Maintain patient records in accordance with the clinics requirements.
Successfully complete the Military Treatment Facility (MTF) provided in-services on intravenous (IV) line placement prior to performance of services.
Actively participate in MHP Team huddles.
Provide evidenced based care within the MHP Team using agreed upon protocols and guidelines.
