OVERVIEW:Reportingto the Southern Care Program Manager the Southern Care Case Manager will beresponsible for providing support to the Sacred Care homes. The Southern CareCase Manager will work within a team that serves to provide comprehensive casemanagement services within a holistic circle of care.
KEY RESPONSIBILITES:
Facilitate pre-service orientation fornew Care Providers.
Establish a positive and supportiverelationship with Care Providers youth in the homes and the support teamthrough being a regular visitor to the home and maintaining ongoingcommunication by phone and via email.
Conduct intake care planning meetingswith guardian agency staff and Care Providers and conduct regularly scheduledcare planning update meetings.
Based on your ongoing awareness of andconnection to the homes complete ongoing care home documentation and assessand complete required licensing documentation:
o HomeStudies and Foster Home License Applications as needed.
o AnnualReviews
o Addendumsas needed.
o FosterCare Licensing Closures as needed.
Maintain an accurate understanding ofthe services provided in each home and effectively communicate with the ProgramManager and guardian agency.
Collaborate with guardian worker indelivering care plans for each child and/or family while remaining focused onchilds wholistic needs.
Collect and review monthly reports andincident reports for each home identify challenges and strategize with thehome and care teams to implement solutions.
Liaise with Care Providers to ensureinformation is effectively communicated.
Communicate to Program Manager theobserved training needs for House Parents Support & Respite Workers andfind those training opportunities.
Contribute to organization committeesbased on your personal and professional experience and goals.
Regular travel is required for thisposition (Winnipeg and surrounding areas) travel and accommodation provided.
Ability to lift and carryitems to a maximum of 50lbs.
Other duties as assigned.
QUALIFICATIONS:
BSW degree or equivalent (similar education and experience working inthe field will be considered).
Knowledge in the areas of the impacts of colonization andintergenerational trauma
Excellent conflict resolution skills.
Knowledge of the Child and Family Services (CFS) sector and legislation.
Good understanding of foster care standards and related licensingpaperwork.
Demonstrate knowledge and/or understanding of Indigenous perspectivesFirst Nations culture and traditional philosophies.
Experience developing and supporting a strong respectful team-basedapproach to service delivery.
Experience working with families and navigating complex situations.
Must have a clear Child Abuse Adult Abuse Criminal Record check (withVulnerable Sector) and Prior Contact Check.
Strong proficiency in Microsoft Office word processing (MS Word)Spreadsheets (Excel) E-mail (Outlook) and Internet.
Excellent communication organization problem solving and team skills.
Ability to speak Anishinaabe is an asset.
Must have a validclass 5 drivers license and access to a vehicle; satisfactory drivers abstractprovided annually.
