Country Manor Home Health Care strives to provide superior health care and services by assisting clients to attain their maximum potential for wellness and independence. As an RN Visit Nurse you can directly help to enrich the quality of life for the individuals and families we serve.
As a Visit Nurse you will:
- Facilitate Personalized Plans of Care: Work closely with clients and their families to develop and implement individualized care plans that address their unique needs and preferences.
- Perform Skilled Nursing Services: Provide expert nursing care ensuring that clinical procedures and protocols are followed to maintain the highest standards of healthcare.
- Collaborate with Interdisciplinary Teams: Engage in effective communication and collaboration with therapists and other healthcare professionals to enhance the overall well-being of our clients.
Qualifications:
- 1 year of nursing experience preferably in Home Health Care
- Broad knowledge of nursing and competence in the area of Home Health Care
- Visual and aural acuity to detect changes in clients
- Strong written and oral communication skills
- Ability to recognize and understand client needs
- Willingness to assume responsibility for caring for and serving the needs of clients in a manner conducive to their safety and comfort
- Up-to-date knowledge of current trends and new developments in nursing
- Ability to work effectively with others in a cooperative considerate and helpful manner
- Reliable transportation
- Willingness to conform to and follow the philosophy and objectives of Country Manor Home Health Care and understand the organizational structure
- Patience consideration and tact when dealing with the client family and community contacts
Education:
- Must be a graduate of a state-accredited school of nursing
- Must be CPR certified.
- Must meet educational requirements to maintain certification
- Hold and maintain a license granted by the State of Minnesota
