Position Overview:
We are seeking a dedicated empathetic and licensed Social Worker to join our growing telehealth team. The ideal candidate will be responsible for supporting patients remotely by providing case management crisis intervention mental health counseling and community resource coordination. This position is a vital part of our care team and contributes to the overall emotional psychological and social well-being of our patients.
Key Responsibilities:
Conduct virtual psychosocial assessments and develop individualized care plans
Provide telehealth counseling services for individuals families and/or groups
Offer crisis intervention and support to patients in distress
Coordinate referrals to community resources including housing food financial assistance and mental health services
Collaborate with physicians nurses therapists and other professionals to ensure continuity of care
Maintain detailed and timely documentation of patient interactions and interventions in the electronic medical record (EMR) system
Monitor patients progress and adjust care plans as needed
Advocate for patients needs within the healthcare system and broader community
Stay current on relevant laws policies and best practices related to social work and healthcare
Qualifications:
Education: Masters Degree in Social Work (MSW) from an accredited institution
Licensure: Active LCSW LMSW or equivalent state licensure (multi-state licensure a plus)
Experience: Minimum of 2 years of post-graduate clinical social work experience preferably in a healthcare or behavioral health setting
Technical Skills:
Proficient in using telehealth platforms and electronic health records (EHR/EMR)
Comfortable with remote communication tools (Zoom Microsoft Teams etc.)
Interpersonal Skills:
Strong communication organizational and time management skills
Ability to work independently and as part of an interdisciplinary team
Cultural competence and ability to work with diverse populations
Work Environment:
100% Remote
Flexible schedule with availability during business hours (Eastern Standard Time preferred)
Reliable internet connection and a private professional work environment required
Work Authorization Requirement:

U.S. Citizens or
Authorized to work in the United States with a valid U.S. work permit.
