Overview
Join our clients team as a Physician for our clients PACE participants. You will play a crucial role in delivering holistic care to high-risk seniors and frail older adults ensuring their well-being and independence.
Key Responsibilities
- Deliver direct primary medical care to PACE participants including history and physical exams. Grow and manage your own participant panel in close collaboration with a complex care RN.
- Initiate evaluate review and close orders for medications diagnostic studies referrals and treatments to ensure high-quality patient-centered care.
- Provide care where participants need it most whether in the PACE center at home virtually or in skilled nursing facilities (SNFs).
- Liaise with IDT members including behavioral health and pharmacists to coordinate comprehensive care plans for each participant.
- Serve as an integral member of the Interdisciplinary Team (IDT) completing assessments participating in care planning and reviewing Service Determination Requests.
- Gain exposure to Value-Based Care strategies and learn how to effectively manage high-need high-cost populations.
- Take after-hour calls remotely on a rotating schedule supported by a nurse triage team.
- Make clinical decisions and coordinate care across disciplines
- Educate caregivers and manage referrals to specialists
- Ensure accurate EMR documentation
Schedule and Shift Details
- M-F 8a-5p. Flexible schedule with on-call shifts. Must be willing to travel to patients homes SNFs/ALs Hospital etc.
- Occasional travel to patients homes SNFs/ALs Hospital etc.

Qualifications
Required
- Doctorate degree
- 1 years of experience in ANY of the following:
- Geriatric Care
- Senior Care
- Geriatric/LTC
- Proof of a valid California drivers license personal transportation a clean driving record and auto insurance
- Physician (2 years)
- ALL of the following valid licenses/certifications:
- Medical Doctor (MD) in California (CA)
- Drug Enforcement Administration (DEA) in California (CA)
- Controlled Dangerous Substances (CDS)
- American Heart Association Basic Life Support (AHA BLS)
- Board Certified (Family Med or Internal Med)
- PACE knowledge is important for this position did the recruiter review PACE verbiage
Preferred
- PACE Experience (1 years)
- Bilingual in Spanish
