Days (United States of America)PuThe Community Health Worker (CHW) serves as a trusted liaison between health and Bon Secours Maryview Medical Center and the community. This role involves providing culturally appropriate health education outreach support and advocacy to improve access to services and promote healthy behaviors in underserved populations. Assists with marketing programs and services as well as data collection.
Build and maintain trusting relationships with community members.
Conduct outreach to identify individuals and families who may benefit from health and social services.
Provide basic health education and information on available resources.
Assist individuals in navigating health systems including scheduling appointments accessing insurance and understanding care instructions.
Advocate for individuals and communities to receive needed services and support.
Collect and report data to monitor progress outcomes and community needs.
Collaborate with healthcare providers social service agencies and community organizations.
Participate in training and professional development to maintain knowledge and skills.
Drives own car to travel to a variety of community locations to provide services.
Required Minimum Education:
High School Diploma or GED
Preferred Education:
2 Year/ Associates Degree-Public Health or Social Work
Licensure/Certification Required:
BLS
Licensure/Certification Preferred:
Certified Community Health Worker
Minimum Years and Type of Experience:
Experience working with target population and with diverse community groups.
Other Knowledge Skills and Abilities Required:
Reliable transportation to drive self to community locations.
