Job Description
Mobile Health Team Inc. a staffing agency has an opening for a Registered Nurse - Case Manager. We are looking for someone with a passion for people.
Location: Tuba City AZ 86045
Assignment Start Date: 7/28/2025
13 weeks Varied 8-hour shift 40 hours weekly

Please get in touch with a recruiter for full details.The Social Worker/RN Case Manager is responsible for utilization review of patient cases in the inpatient and outpatient services departments and review of medical necessity of referrals to specialty providers for services not normally provided within the facility. The incumbent will work with a multidisciplinary team to promote the efficient and effective use of a variety of healthcare resources by focusing on the whole patient to provide a seamless continuum of care. The goals are the provision of quality care decrease the fragmentation of care enhancement of patient self-care and quality of life and cost containment. The incumbent will work closely with Providers Nurses Social Workers Physicians the PRC department and all health care providers staff to ensure implementation of appropriate resources in the fulfillment of utilization goals for the hospital and third-party payors. The Social Worker/RN Case Manager will be responsible to the Director of Case Management/Care Coordination.
EXPERIENCE/EDUCATION/QUALIFICATION
Experience:
- Must have a minimum of one (1) year of experience in the clinical care setting
- Experience and knowledge of discharge planning case management or utilization review processes.
- Demonstrate knowledge of electronic health record systems.
Education:
- Associate degree in Nursing
- must obtain a bachelors in nursing within 2 years of hire.
Licenses Certifications and/or Registration:
- Must have and maintain current Basic Life Support (BLS) certification by the American Heart Association throughout employment
- A valid current full and unrestricted Professional Nursing License to practice nursing in any state of the United States of America The Commonwealth of Puerto Rico or a territory of the United States
EMR System: Allscripts experience preferred
Shift: Monday to Friday 8 am-5 pm & Weekend rotation with call
Vaccinations: COVID-19 vaccination is MANDATORY and workers must be fully vaccinated 2 weeks before the start date. Declination is considered for Medical reasons ONLY with appropriate documentation and is subject to confirmation & review.
***Fingerprinting must be submitted and cleared before start!*
ORIENTATION: Every two (2) weeks only.
HOUSING APPLICATION: must be submitted 2 weeks before the start date
