Job Description
- Evaluate diagnose and treat adult patients with communication cognitive and swallowing disorders
- Develop and implement individualized patient-centered treatment plans
- Work collaboratively with an interdisciplinary team (PTs OTs RNs physicians)
- Document patient care progress and outcomes using Cerner and UDS EHR systems
- Educate patients families and staff on communication and swallowing strategies
- Participate in care planning and discharge recommendations
- Serve adult patients in an acute inpatient rehabilitation setting (40-bed unit)
- Manage cases with common diagnoses such as neurological conditions Parkinsons disease and general debility
- Maintain high standards of clinical care for neuro and debility-related conditions
- Contribute to a supportive collaborative and efficient team culture
