Job Description
- Provide hands-on care and daily living assistance to adult and geriatric patients in an inpatient rehabilitation setting.
- Support patient mobility including transfers and ambulation ensuring patient safety at all times.
- Monitor and document vital signs intake and output reporting any changes in patient condition to nursing staff.
- Offer emotional support and encouragement to patients recovering from serious illnesses injuries or surgeries.
- Assist nursing and therapy staff with patient transport and preparation for treatments.
- Maintain a clean safe and therapeutic environment for all patients.
- Accurately document care provided and patient observations in the electronic health record system.
- Work with a range of patient diagnoses including stroke orthopedic injuries brain/spinal cord injuries and neurological/trauma conditions.
- Collaborate effectively with healthcare team members to deliver high-quality patient care.
- Flexible per diem schedule with day (6:45 AM 7:15 PM) and night (6:45 PM 7:15 AM) shifts available including weekends and holidays.
- Must hold active CNA certification and BLS certification; prior rehabilitation or acute/skilled nursing experience preferred.
