Job Summary:
This Licensed Clinical Social Worker (LCSW) position requires providing comprehensive psychosocial support to patients within a short-term acute care facility. The role involves floating between units conducting risk assessments facilitating psych placements offering substance use disorder (SUD) support utilizing social determinants of health (SDOH) resources and collaborating with the interdisciplinary team. Weekend work is required with the schedule varying based on PTO.
Location: Medford Oregon United States
Responsibilities:
- Conduct comprehensive psychosocial assessments of patients.
- Develop and implement individualized treatment plans addressing patients psychosocial needs.
- Facilitate psych placements and coordinate care with external resources.
- Provide SUD support and resources to patients.
- Utilize SDOH resources to address patients social needs impacting their health.
- Collaborate effectively with the interdisciplinary team including physicians nurses and other healthcare professionals.
- Maintain accurate and timely documentation of patient interactions and progress.
- Perform risk assessments to identify and mitigate potential safety concerns.
- Participate in unit meetings and contribute to the overall improvement of patient care.
- Float between various units as needed.
Required Skills & Certifications:
- Active LCSW license in the state of employment.
- Strong understanding of psychosocial issues affecting patients in acute care settings.
- Experience with risk assessment psych placements SUD support and SDOH resources.
- Excellent communication interpersonal and collaboration skills.
- Ability to work effectively in a fast-paced environment.
- Proficiency in electronic health record (EHR) systems (Epic preferred).
Preferred Skills & Certifications:
- Experience in an acute care or long-term acute care hospital (LTACH) setting.
- Experience with a diverse patient population.
- Bilingual skills (specify languages).
- Experience with specific treatment modalities (e.g. CBT DBT).
Special Considerations:
- Weekend work is required with the schedule varying based on PTO.
- No on-call requirements.
- Floating between units is required.
- Patient ratios are approximately 1:40.
- 48-hour workweeks are not approved.
Scheduling:
- Variable schedule including weekend work depending on PTO.
- No block scheduling.
- Shift assignments will be determined based on unit needs and staffing levels.
