Job Summary:
This Registered Nurse position requires providing skilled nursing care case management and hospice care to patients facing advancing age frailty and serious illness. The role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential.

Responsibilities:
- Patient assessments and monitoring.
- Medication administration and documentation.
- Treatment planning and implementation.
- Collaboration with physicians other nurses and healthcare professionals.
- Case management and care coordination.
- Providing skilled nursing care including hospice care.
- Maintaining accurate patient records and documentation.
- Participating in interdisciplinary team meetings.
- Adhering to all hospital policies and procedures.
Required Skills & Certifications:
- Clinical assessment medication administration and treatment planning skills.
- Active Registered Nurse license in the state of employment.
- Bachelors degree in Nursing or related field.
- Minimum two years of experience in hospital home health or hospice care.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Drivers license car insurance and automobile for field work (if applicable).
Preferred Skills & Certifications:
- Experience in Medical/Surgical ICU ER Psychiatric Labor & Delivery PACU or Operating Room settings.
