ESSENTIAL DUTIES FUNCTIONS AND RESPONSIBILITIES
- Works with HPDP staff to collect and review data and assist them to enter data into database management system.
- Develops data spreadsheets analyzes data/trends and reports data in meaningful graphs reports etc.
- Assists HPDP Coordinator and staff to develop and implement strategies; plan for meeting HPDP/HLO program goals strategies and objectives.
- Assists in developing HPDP program baseline measures and targets.
- Tracks the progress of improvement initiatives/projects/Plan-Do-Study-Act (PDSA) and compiles the results of tests for change and generates reports to HPDP Coordinator.
- Researches scientific journals (School Health Journal Health Promotion Journal Community Mental Health Journal etc.) to find best practices and evidence-based programs models and strategies.
- Collaborate coordinate plan implement and evaluate HPDP projects as requested.
- Assists HPDP Coordinator with generating monthly quarterly and annual reports/graphs for the HPDP program.
- Research plan and develop health presentations to local schools chapters and other organizations.
- Submits monthly HPDP Department QAPI Report (Quality Assurance Performance Improvement) to FDIHB Quality Services Division.
- Assists HPDP Coordinator and HPDP Facilitator in developing short/long range goals objectives and measurable targets.
- Supports HPDP improvement team through advising coaching and training on improvement methods measurement data collection and reporting.
- Plans designs and implements HPDP program surveys/questionnaires to improve quality and monitor performance.
- Establish relationships with local Tribal State regional stakeholders to collaborate and facilitate public health program initiatives/partnerships.
- Analyze existing or proposed systems strategies services or other health-related matters for long range implementation and administration of the public health programs.
- Plans and organizes improvement training and development for HPDP staff.
- Performs ongoing evaluation of HPDP projects and reports on progress and gives recommendations/feedback.
- Works with local Navajo Chapters to assist with Community Health/Wellness Planning & Projects (Healthy Dine Nation Act of 2014 / Junk Food Tax) including walking trails gardens greenhouses farmers markets nutrition playgrounds fitness centers training and related activities.
- Utilizes various public health planning and performance improvement tools such as the Ecological Model Health Literacy Health Communication/Infographics marketing campaigns improvement process PDSA Fishbone Diagrams Run/Control Charts Flowcharts Force Field Analysis Histograms Logic Models Pareto Charts Radar Charts/Graphs and Gantt Charts.
- Performs other duties as assigned.
MANDATORY MINIMUM QUALIFICATIONS
Experience: One (1) year experience working with community health programs schools Navajo Chapters and related organizations.
Education: Bachelors Degree in Public Health Health Promotion Community Health Health Education Behavioral Health Social Work Substance Abuse or related field.
