Job Description
Community Health Care Coach
Salary: $39000
Elder Options is seeking a Community Health Care Coach to provide community health education follow up support and coaching to patients. Services include Care Transition services supporting hospital to home transitions and/or other community-based health and wellness services. The interventions are geared toward patient self-management of chronic diseases via teaching/modeling self-advocacy and education with the purpose of reducing re-hospitalizations providing individualized assessments care planning referrals and supports to adults with health and wellbeing goals to support their independence in the community. Home visits are required and remote work is limited.
