Roles & Responsibilities
My client is seeking an experienced healthcare professional to lead a team focused on post-discharge care coordination and community integration. This role involves strategic planning team leadership and cross-sector collaboration to improve patient outcomes and reduce unnecessary hospital readmissions.
Key Responsibilities:
- Lead and mentor a team of care coordinators supporting patients and caregivers after discharge
- Oversee follow-up care through phone outreach and home visits ensuring patients can safely transition back into the community
- Coordinate with healthcare partners to streamline services and strengthen support systems
- Develop and implement frameworks that enhance community care delivery
- Supervise clinical performance and guide professional development for team members
- Monitor data quality and contribute to service improvement initiatives
- Participate in internal projects to improve care standards and operational efficiency
Requirements:
- Professional background in Nursing Social Work Allied Health or related clinical fields
- Minimum 8 years of experience in care coordination or community health
- Proven leadership experience (35 years) managing multidisciplinary teams
- Strong communication planning and stakeholder engagement skills
