About the role
Take the lead in coordinating patient flow for the Transitional Aged Care Program (TACP) working closely with referrers and stakeholders to ensure timely effective care
Be the key link between hospital community and aged care servicesstreamlining referrals and intake to support seamless transitions for older adults
Use your advanced clinical expertise to assess patients across hospital and community settings supporting TACP clinicians with complex cases
Partner with the TACP Manager to shape service deliverymanaging team caseloads contributing to case conferences and embedding wellness and reablement principles
Drive innovation by leading quality improvement initiatives and helping develop policies that enhance care outcomes across TACP and CHSP
Share your knowledge and leadershipsupervising senior Allied Health staff and representing SVHNS at key forums committees and panels
About you
Youre deeply aligned with the values and mission of Mary Aikenhead Ministries and St Vincents Health Australia bringing integrity to everything you do
You hold current AHPRA registration and are confident navigating the complexities of aged care in the community
Your communication and leadership skills shine as you collaborate effectively within multidisciplinary teams
Youre experienced in driving service improvements and redesigns that make a real difference to patient care
You understand the ins and outs of funding service agreements and targets within the Transitional Aged Care Program ensuring seamless program delivery
