Position Overview

We are seeking a Full-Time Admissions Coordinator to join our team at The Grand Rehabilitation and Nursing at River Valley.

As the first point of contact for incoming residents and families youll play a key role in ensuring a smooth admissions process while building strong relationships with referral sources. This role blends customer service healthcare coordination and marketing to support census growth and an outstanding guest experience.

Responsibilities

Admissions Coordination

  • Respond promptly to inquiries and referralsideally within one hour.

  • Review PRI (Patient Review Instrument) and collaborate with clinical teams for admissions decisions.

  • Coordinate all aspects of the admissions process including pre-admission preparation and welcome procedures.

  • Conduct in-person welcome meetings with new residents and their families.

Referral & Relationship Management

  • Develop strong partnerships with hospitals physicians discharge planners and social workers.

  • Provide timely updates to referrers and stakeholders.

  • Help meet or exceed facility census targets through proactive outreach.

Marketing & Guest Experience

  • Support community outreach and facility marketing initiatives.

  • Ensure a guest-centered admission experience that promotes satisfaction and referrals.

  • Represent The Grand professionally in the community to maintain and build a positive public image.

Requirements

  • Experience with PCC FCC Portal and Census Pro (preferred).

  • Excellent communication and interpersonal skills.

  • Proven ability to manage priorities and make independent decisions.

  • Skilled in MS Office (Word Excel Outlook).

  • Prior experience in long-term care or healthcare admissions strongly preferred.
