PAY RATE: 75k-80k/annually

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provide clinical care through assisted living and memory care protocols
  • Perform and document resident assessments and complete Individualized Service Plans
  • Provide or delegate hands-on clinical care per care plans; evaluate resident outcomes
  • Collaborate with physicians pharmacists and other providers for coordinated care
  • Conduct skill evaluations; make daily rounds to review care team member performance
  • Preserve resident retention through superior customer service
  • Investigate and resolve resident and family concerns
  • Lead and manage clinical quality and process improvement initiatives
  • Recruit hire train schedule and evaluate clinical staff; ensure regulatory compliance
  • Maintain high team member morale and foster a supportive environment
  • Provide immediate and courteous assistance to residents families and visitors
  • Gather and review medical and contact information for all new residents
  • Responsible for overall safety of residents and the community
  • Collaborate with Sales partners to assess potential residents and community capability
  • Assist with resident transfers and move-outs
  • Conduct timely assessments to ensure assisted living and memory care needs are met
  • Track trend and report clinical quality data to identify risk
  • Provide oversight of the medication management program to promote resident safety
  • Investigate incident reports and ensure completion of resident status documentation
  • Maintain accurate charts and state-required documentation; prepare for health inspections
  • Ensure compliance with Federal State/Provincial board of nursing and other regulations
  • Communicate regularly with other disciplines involved in patient care including weekends
  • Complete all assigned duties as needs and circumstances evolve
  • Serve as a member of the communitys emergency response team
  • Maintain professional appearance and hygiene in accordance with company policy
  • Operate and maintain equipment inventory and supplies according to established procedures

Minimum Requirements

  • College degree required; graduate of an accredited LVN/LPN/RN program preferred
  • 23 years of experience in assisted living or memory care
  • Experience in geriatric nursing emergency care and first aid
  • If licensed must hold a current unencumbered RN or LVN/LPN license in Texas
  • Current CPR certification and state-mandated Continuing Education credits
  • Valid Drivers License
  • Computer proficiency with EMRs Microsoft Office and the ability to learn new systems
  • Strong communication and customer service skills
  • Flexible availability across hours shifts and days
