JOB SUMMARY:
The Community Health Worker screens assesses and links adults and families to healthcare benefits/entitlements nutritious food affordable housing and other needed social and community supports.
ESSENTIAL JOB FUNCTIONS: List all essential job duties. Conducts outreach and engagement to Medicaid recipients in ICL and the community
Conducts screenings to confirm unmet HRSNs
Develop social care plans
Navigate services in the Uniteus platform to existing federal state or local services or refer to enhanced HRSN services
Confirm service delivery completion
Follow up with members to determine needs being met
Communicates effectively with people served providers community organizations and members of the community
Maintains documents records statistics and other related reports in an organized timely and accurate manner as per policy and procedure.
Provides telephonic as well as face-to-face outreach engagement and service planning in the field.
Provides crisis intervention and follow-up.
Perform other related duties that may be assigned.

QUALIFICATIONS AND EXPERIENCE:
A bachelors degree (preferred not required) plus one year providing direct services to people with Serious Mental Illness developmental disabilities or substance use disorders; or in linking individuals with Serious Mental Illness developmental disabilities or substance use disorders to a broad range of services essential to successful living in a community setting (e.g. medical psychiatric social educational legal housing and financial services).
