Transitional Support Worker
Full Time: Fixed Term Contract
Location: Hunter & Central Coast
This role is paid under the Social Community Home Care & Disability Services Award 2010- Level 5.1
The Opportunity:
The Premiers Youth Initiative (PYI) aims to prevent homelessness among young people leaving care by diverting them from the homelessness service system before they become homeless or at risk.
The role of Transitional Support Worker will be to provide transitional support to young people in targeted locations to build their capacity to establish and maintain long term accommodation with a particular emphasis on building capacity to establish and maintain share accommodation over time through up to 4 years transitional support.

Career and development opportunities
The Essentials:
Tertiary Certificate in Youth Work/Community Welfare/Social Work
Excellent knowledge understanding and experience of working from a trauma informed strengths-based approach with young people who have lived in Out of Home Care
Experience providing incidental counselling and mentoring to young people with complex needs
Experience in building relationships networking working cooperatively with referring to and advocating with a range of diverse services that offer supports to young people
Well-developed knowledge of diverse cultures and community needs and issues as they relate to care of children and young persons so as to effectively manage reports of risk of harm to children and young people
Experience and highly developed skills in goal setting and action planning for young people leaving Out of Home Care
Knowledge and understanding of the rights legislation and resources available to children and young people who have been placed in Out of Home Care including requirements for leaving and aftercare planning
Ability to utilise confidentiality principles prioritise competing demands and meet deadlines while working independently
High level of planning and organizing skills to support allocated clients together with investigative assessment problem solving and analytical skills.
The successful applicant will possess or willingness to obtain the following: NSW Working with Children Check Criminal History Check first aid & CPR certificate and undertake a pre-employment functional medical.
