Job Description

Make a Difference in Mental Health Care

Are you passionate about helping individuals and families navigate complex systems of care We are seeking a dedicated and organized Patient Care Coordinator to join our inpatient care team. In this vital role you will serve as a central connection point between patients families treatment teams and community resources ensuring coordinated compassionate care throughout the patient journey.


Key Responsibilities:

  • Manage a caseload of inpatient and/or residential clients
  • Collaborate with interdisciplinary teams to develop implement and update treatment and crisis plans
  • Act as a liaison between patients families schools healthcare facilities and community agencies
  • Help families access mental health educational and community-based services
  • Monitor delivery of care track progress and support discharge and aftercare planning
  • Ensure all documentation billing and regulatory requirements are met
  • Identify and address cultural social and systemic needs or barriers to care
  • Participate in treatment meetings rounds and care coordination activities

Requirements

Required Qualifications:

  • Bachelors degree in a human services field
  • At least 1 year of full-time experience working with individuals with mental illness

Preferred Qualifications:

  • 2 years of experience in case management or healthcare/social services
  • Experience in an inpatient or residential care setting
  • Familiarity with Medicaid IEPs mental health systems and community resource navigation
Salary Description
Starting at $23.65/ hour!
