Job Description
Make a Difference in Mental Health Care
Are you passionate about helping individuals and families navigate complex systems of care We are seeking a dedicated and organized Patient Care Coordinator to join our inpatient care team. In this vital role you will serve as a central connection point between patients families treatment teams and community resources ensuring coordinated compassionate care throughout the patient journey.
Key Responsibilities:
- Manage a caseload of inpatient and/or residential clients
- Collaborate with interdisciplinary teams to develop implement and update treatment and crisis plans
- Act as a liaison between patients families schools healthcare facilities and community agencies
- Help families access mental health educational and community-based services
- Monitor delivery of care track progress and support discharge and aftercare planning
- Ensure all documentation billing and regulatory requirements are met
- Identify and address cultural social and systemic needs or barriers to care
- Participate in treatment meetings rounds and care coordination activities

Required Qualifications:
- Bachelors degree in a human services field
- At least 1 year of full-time experience working with individuals with mental illness
Preferred Qualifications:
- 2 years of experience in case management or healthcare/social services
- Experience in an inpatient or residential care setting
- Familiarity with Medicaid IEPs mental health systems and community resource navigation
