About the Role:

The Home Care Coordinator is responsible for delivering client directed care services to home care package clients in line with the Aged Care Quality Standards. The HCP coordinator will plan develop and implement individualised client care plans that support the client to live safely and independently in their own home.

The key objectives of the role include:

  • Accept referrals from the My Age Care Portal to ensure clients are assessed for appropriate needs and goals care plans and budget in line with guidelines.
  • Undertake client reviews on an annual basis or as required
  • Provide case management including co-ordination of services case conferences with client and family when required to understand and interpret budgets and funding agreements.
  • Promote Biripi ACMC Home Care Package services to potential and existing clients and their families.
  • Actively contribute to maintaining clients home and social environments.
  • Liase with clients family members General Practitioners and other health care providers to identify clients individualised goals to develop and review clients care plans.
  • Ensure services are provided in a manner that promotes the personal values of the client which adhere to relevant Codes of Ethics Standards and Legislation relating to professional practice and the Home Care Package guidelines.
  • Assist clients in meeting their needs in accordance with their care plan and goal planning using a Consumer Directed approach.
  • Assist with the preparation management and monitoring of client budgets in relation to care provision.
  • Develop close working relationships with relevant services community groups and organisations.
  • Actively collaborate with multidisciplinary teams to ensure that clients care plans are relevant and appropriate.
  • Ensure care plans are being supported by Home Support Workers

Essential Criteria:

  • Certificate IV in Case Management or Community Services or related discipline or willingness to obtain within 6 months.
  • Experience in coordination of Home Care Packages and/or Home and Community Care program
  • A Commitment to working with our clients and community to engage in Consumer Directed Care.
  • Ability to undertake assessment planning developing implementation evaluation and documentation of quality client care
  • Ability to work with Aboriginal and Torres Strait Islander communities and their leaders respecting traditional culture and ways of doing business.
  • Demonstrated cultural awareness and sensitivity and the ability to build and maintain strong relationships with the local community.
