Position Title: Case Manager Neighbors First Families

Reports to: Case Manager Supervisor/ Division Director

Location: Washington DC 20009

Hours: Monday-Friday (9am-5pm); Some evenings holidays and weekends may be required to support operations and respond to emergencies

Status: Regular Full-Time (Exempt)

Friendship Place offers the most effective model for addressing homelessness with innovative customized programs that have positive demonstrable results and a lasting impact on the community and beyond. Our programs are person-focused - individualized to meet the needs and goals of individuals families youth/young adults and Veterans experiencing homelessness. We provide outreach hospitality health care case management housing job placement services and advocacy.

Summary: The position provides permanent supportive housing services to families experiencing homelessness identified as the most vulnerable in the District of Columbia. The Neighbors First Families Program follows a Housing First model and philosophy. The Case Manager works to empower participants to rebuild their lives relying on their own strengths and using wraparound resources in a holistic participant-driven process.


Essential Duties and Responsibilities:

  • Provide supportive services to families experiencing homelessness with serious mental illnesses addictions medical concerns etc.
  • Deliver face-to-face contacts to consumers and their children spending approximately 80% of time in the field
  • Demonstrate empathy and flexibility in responding to consumers needs and requests fostering wellness and independence
  • Engage consumers and assess individual needs preferences and strengths
  • Develop implement and revise individual service plans
  • Assist households in locating and selecting wraparound services
  • Monitor medical/psychiatric needs and medication management of all household members
  • Assist participants in securing adequate housing resources
  • Act as a liaison with landlords and other community stakeholders
  • Participate in multidisciplinary meetings and regular supervision
  • Collect outcome statistics and write monthly progress reports
  • Maintain participants electronic records in the Housing the Homeless database and physical files
  • Attend professional trainings and conferences as needed
  • Work collaboratively with clients colleagues and external service providers utilizing strong interpersonal and intrapersonal communication skills to support a positive solution-oriented work environment and to build trust resolve conflicts and ensure coordinated client-centered care.
  • Actively engage in continuous personal and professional development by seeking out training feedback and learning opportunities to enhance skills knowledge and effectiveness in the role
  • Must maintain compliance with background check deadlines and all assigned mandatory training
  • Perform all other duties as assigned
Requirements

Education/Experience:

  • A High School diploma or GED and six years of direct human service experience providing counseling and/or case management services to vulnerable or at-risk populations OR
  • A bachelors degree in any field and two years of direct human service experience providing counseling and/or case management services to vulnerable or at-risk populations OR
  • A masters degree in social work or a related a human services field and one to two years of experience

Background & Qualifications:

  • Must have or be able to obtain a First Aid/CPR/AED certification that includesa hands-on skills testing module. May be called into action to perform life-saving measures. First Aid/CPR/AED training will be provided by Friendship Place
  • Understanding of adult rehabilitation services
  • Experience with families youth and school systems
  • Experience with addictions mental health and co-occurring disorders
  • Strong written and verbal skills
  • Must have strong intra- and interpersonal communication skills with the ability to communicate professionally and effectively both in writing and verbally.
  • Ability to work in a culturally diverse environment
  • Ability to work well under pressure
  • Ability to work as a team player

Working conditions: Position will spend time between office/desk settings traveling to meet with households across the service area and attend meetings/engagements with key stakeholders. Field time consists of to up to 80%. Driving time for position responsibilities may consist of up to 25%-40%


Physical and Environmental Requirements: Position requires split time between an office setting and traveling to engagements via car or public transportation. Employees must be able to sit for extended periods of time with repetitive motion and positions. Employees must be able to operate a motorized vehicle. Some lifting may be required (up to 20-25lbs)


Background checks: This position is classified as Safety Sensitive under DHS. The selected candidate will be required to complete and pass background checks specific to contract requirements that may include criminal history background check and fingerprinting FBI Background Check DC Metropolitan Police Background Check State-of-Residence Police Background Check DC Child Abuse Registry (CFSA) National Sex Offender Registry Check Drug Panel and Alcohol Screening Tuberculosis Test and Driving Record check (for positions transporting participants). Reference checks and verification of current and/or previous employment are required

Specific Requirements: Must have his/her own transportation to carry out job duties and responsibilities. The vehicle must meet legal requirements (insured registered etc.) and the employee must have a valid drivers license


Compensation: The hiring pay range is between $58117.00 - $63506.00. The starting annual salary is based on a step system. A candidates starting pay determination is made based on meeting the basic position requirements and current completed education credentials and work experience. Please Note: The step system is set and not open to pay negotiation.
