PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Housing & Landlord Partnership team as the RPSS Residential Service Coordinator at the RHC Office.

Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.
ABOUT THE JOB
As part of the Housing & Landlord Partnership team the RPSS Residential Service Coordinator works as part of the case management team to provide supportive services to RPSS tenants and assists them in breaking the cycle of homelessness. As part of this team the RSC supports residents transition from street to housing by identifying their individual needs and utilizing a coordinated system to facilitate and ensure their ability to access and maintain identified necessary services. Focusing on housing and health as necessary outcomes for success the RSC works with community partners to address barriers assist with maintaining and sustaining housing and supporting health needs.
Position Responsibilities include:
- Coordinate move-in into housing units.
- Serve as a liaison between tenant and community-based case management team.
- Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks.
- Provide assistance for critical issues that impact housing stability.
- Respond to housing and tenant-related emergencies during normal business and after hours.
- Identify and secure community resources in coordination with assigned case manager.
- Gain expert knowledge of homeless populations common barriers to housing and overall knowledge of community resources.
- Understand the building lease term agreement and provide education to tenants and case managers on lease requirements.
- Maintain client related data tracking systems including case notes and complete HMIS and Salesforce entries.
- Prepare case-related reports including but not limited to outcomes successes and challenges.

PREFERRED QUALIFICATIONS
- Two (2) years of experience in a human services or behavior-related field.
MINIMUM QUALIFICATIONS
- All levels of experience and education welcome
MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:

- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:
- Have reliable transportation
- A valid drivers license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATHs insurance coverage
