SUMMARY

Support Medical and Dental Case Management by assisting with the coordination of dental care to include liaising with drill weekend military personnel for continuity of care and transition to the week operations and to coordinate dental treatment and referrals while working in collaboration with Health Systems Support (HSS) team for soldier care driven functions.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Coordinate care for those soldiers identified as DRC3 schedule appointments assist the records room personnel with administrative tasks and ensure all treatment plans are completed in a timely manner.
  • Responsible for tracking medical/dental status of all assigned Soldiers; including compliance and non-compliance to meet the ultimate goal of maximizing Soldiers returning to duty and increasing deployability.
  • Responsible for medical charting in eCase. Responsible to access and utilize: MODS eProfile MHS Genesis MWDE MEBPrep QTC Portal MEDPROS; and MEDCHART: DenClass eCase HRR DOEHRS hearing management repository MRR BH case management tracking records input reports voucher management and case documentation.
  • Request profile exams/Fit for Duty evaluations including: PHA medical behavioral health dental and audiology exams at physical exam center or through RHRP QTC in clinic (voucher) process in the QTC portal or equivalent NGB approved system.
  • Coordinate Soldier treatment plans with appropriate medical facilities; including contracted Dental Services. Will work with identified Soldier and Dental services to schedule and after treatment follow-up on treatment plan as applicable.
  • Track referral in e-case in MEDCHART.
  • Attend PHA events as determined by the OTSS according to the needs of the state and the individual units.
  • Ensure those Soldiers referred for dental treatment are referred tracked and followed up with appropriately.
  • Submit case processing medical documentation and profile concerns to military or contracted Provider(s).
  • Manage Soldiers care/e-Case from the identification of the condition to when the Soldier is Considered medically ready (fit for duty) or administratively separated from the military. 100% of Soldiers undergoing Case Management will be contacted at a minimum once monthly. Contact is defined as a telephonic conversation (TCON) E-mail communication where the Soldier responds to CM message or a face-to-face encounter.
  • Follow the regulations and policies applicable to Individual Medical Readiness (IMR) and deployment to include the AR 40-501 and other applicable regulatory policy and guidance.
  • Ensure documentation received from the Soldier and/or their medical/dental team is received and placed in appropriate electronic record (Denclass and HRR in MedChart) as well as the Soldiers hard copy record.
  • Maintain Soldier medical records in accordance with Army Regulation (AR) 40-66.
  • Provide Records Custodian or delegate with all relevant medical/dental documentation that can liaison between medical events and dental treatment sites and data records site.
  • Obtain medical release of information from Soldier.
  • Educate Soldiers on resources available to improve mental health (LCSWs) and dental readiness (SCC).
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
  • Review and track dental treatment progress.
  • Upload medical or dental documents of case managed Soldiers into Health Readiness Record (HRR).
  • Stamp all medical documents for verification after uploading.
  • Index uploaded medical documents.
  • Schedule service members as appropriate for weekly clinic slots.
  • Provide all medical/dental documents received for case management to Records Custodian to be filed using a document evidencing transfer of custody from the Contractor to the government.
  • Ensure all administrative charting in eCase is completed if indicated.
  • Track medical/dental appointments by communication with the unit and the Soldier to ensure Soldier attendance and submission of follow up documentation.
  • Log in eCase any contact with SM (email voice call or personal interview).
  • Communicate with delegated government personnel to track compliance versus noncompliance.
  • Run MEDPROS and/or MEDCHART report Post PHA/SRP event or as requested by the COR/OTSS.
  • Monitor MEDPROS and MEDCHART to ensure all scheduled services are completed/updated and current medical status is reflected.
  • Report Medical Readiness to the Commander or his/her designated representative.
  • Assist with release of records and records requests by current as well as retired service members.
  • Maintain regular and punctual attendance.
  • Perform other duties as assigned.

Requirements

MINIMUM QUALIFICATIONS

  • Must have the technical skills and expertise necessary to perform duties with minimal supervision.
  • A solid background in office management with at least five (5) years of experience or an equivalent combination of Case Management and/or Care Coordination experience.
  • Must obtain and maintain any necessary security access and/or background checks.

DESIRED QUALIFICATIONS & SKILLS

  • Experience commensurate with office management i.e. a medical/dental office manager medical accounting dental hygiene or dental assistant in conjunction with case management experience.
  • Demonstrates ability to prioritize organize problem-solve and coordinate daily activities to support effective improvement in individual patients.
  • Demonstrates knowledge skill and ability to use basic office automation and information management equipment and programs (i.e. word processing database management email etc.).
  • Excellent patient assessment skills.
  • Sufficient initiative interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients and healthcare staff from diverse backgrounds.
  • Ability to effectively read write and speak English in order to communicate with all patients and other healthcare providers.
  • Excellent written communication skills to perform accurate documentation both written and electronic of all activity in accordance with requirements.

WORK SCHEDULE

Full-time. May be required to work additional hours as needed to complete assignment or project.

TRAVEL

May be expected to travel to locations around the state as needed to support the NMARNG OTSS during Medical Readiness Events. These events typically occur once a month between December and May of each year. The expected travel time is less than 25%.

WORK ENVIRONMENT

Work is primarily performed in a professional office setting including the normal office equipment (e.g. computer fax machines copier) and with a wide variety of people in differing functions personalities and abilities.

PHYSICAL DEMANDS

Position consists of sitting for long periods of time using hands/fingers handling reaching with hands and arms speaking or translating. Periods of walking standing bending kneeling stooping crouching and lifting boxes weighing up to 30 pounds. Moderate to extended periods of office work with heavy periods of computer usage. COMPENSATION & BENEFITS PACKAGE

  • Pay rate: $26.50per hour.
  • H&W: $4.57
