About the Role:
Max-term full-time role delivering high quality evidence-based mental health care and support to clients of the Wesley Mission SHS Team who live with mental illness and are experiencing or at risk of experiencing homelessness.
Deliver comprehensive assessments clinical intervention care coordination and referral on outreach alongside Wesley Mission staff.
Collaborate with clients multidisciplinary team Wesley Mission and stakeholders to address cultural holistic health and support needs using a strengths-based harm reduction approach.
Build strong partnerships and coordinate care with service providers and communities to strengthen client support networks.
Assist clients in accessing mainstream and specialist healthcare support services and stable accommodation to improve health and wellbeing outcomes.
Lead staff education contribute to clinical reviews and drive quality improvement across services.
About You:
Skilled compassionate Social Worker Occupational Therapist or Registered Nurse with current AHPRA registration or eligibility for AASW membership plus post-graduate qualifications.
At least 3 years experience delivering mental health assessment treatment and care coordination to people with complex health and support needs who are also experiencing homelessness.
Strong understanding of homelessness trauma-informed care strengths-based frameworks and committed to clinical supervision and professional development.
Confident senior team member who drives service development education and quality improvement initiatives with excellent communication and interpersonal skills.
Tech-savvy and mobile comfortable with digital health records and holds a current NSW Drivers Licence.
