Summary:

Provides support for all Service Coordination Department functions which include utilization management service coordination and population health management. This position also provides health education and care coordination services for CCHP members telephonically in home or health facility settings.

This role will provide support for all Service Coordination Department functions which include utilization management service coordination and population health management. This position also provides health education and care coordination services for CCHP members telephonically.

Hybrid position: Will work four 10-hour days 7 am-5:30 pm. Will primarily work from home with 2 days per month in office

Required Education Experience & Certification:

  • High School graduate.

  • Three years experience in a managed care or medical office/hospital environment.

  • Bi-lingual (English/Spanish) Required. Expert proficiency in conversational reading writing and translating medical terms as well as diagnoses. Must pass Spanish test prior to hire.

  • Certified Community Health Worker certification required.

Knowledge Skills & Abilities

  • Superior verbal and written communication.

  • Strong customer service skills strong phone skills.

  • Familiarity with basic English & Spanish medical terminology and ICD-9 and CPT coding.

  • Personal computer skills.

  • Expertise with various software applications/databases (i.e. Microsoft Office).

  • Highly organized ability to prioritize and demonstrate moderately complex problem-solving skills.

  • Ability to work autonomously with minimal supervision.

  • Basic service coordination concepts.
