Job Description

KAW NATION POSITION DESCRIPTION
TITLE: COMMUNITY HEALTH REPRESENTATIVE

IMMEDIATE SUPERVISOR: HEALTH DIRECTOR

CONTROLLING SUPERVISOR: CHAIR


QUALIFICATIONS:
1. GED or High School Diploma.
  1. Certification in health-related field is preferred.
  1. Ability to complete health training programs.
  1. Demonstrate awareness sensitivity and appreciation of Native culture traditions and socioeconomic needs.
  1. Current CPR and First Aid certifications.
  1. Must have and maintain a valid driver s license with clean driving record.
  1. Must be able to lift at least 25 pounds and move about with ease.
8. Skill and ability to communicate courteously and effectively orally and in writing
9. Basic knowledge of Microsoft Office software (Outlook Word and Excel).
10. Ability to work independently and productively.
11. Maintain strict confidentiality regarding patient health infom1ation.


RESPONSIBILITIES:

  1. Work in conjunction with providers and clinic staff to improve the health status and health knowledge of the tribal community focusing on tribal elders and children.
  1. Visits clients in their home to perform vital sign measurements diabetic foot exams (visual and sensory) diabetic blood testing and perform minimal patient assessments.
  1. Provide general health education in the use of such things as: walkers crutches wheelchairs and canes; minimal nutritional education and assist clients with referrals.
  1. Provide non-emergency medical transportation for eligible clients.
  1. Provide health promotion/disease prevention activities in collaboration with clinic staff.
  1. Responsible for disbursement and monitoring of medical supplies and pharmaceuticals to clients.
  1. Prepare and submit monthly quarterly and annual reports as required.
  1. Identify and maintain client data base.
  1. Attend necessary trainings to improve and enhance services.
  1. Complete any other duties as assigned by the immediate/controlling supervisor.
