Curant Health is searching for a PRN Clinical Pharmacist to join its team in Smyrna GA.
As the PRN Clinical Pharmacist we will look to you to perform medication-related clinical services including but not limited to delivering Comprehensive Medication Reviews to our Care Management services to patients via outbound and inbound communications that optimize therapeutic outcomes for patients.
Responsibilities
- Provide medication-related clinical services to patients via telephone or other telehealth technologies (such as video conferencing).
- Perform comprehensive medication reviews to collect patient-specific information and assess medication therapies to identify medication-related issues.
- Consult with prescribers/healthcare providers to resolve medication-related issues.
- Create and communicate a Personal Medication Record (PMR) for the patient.
- Create and communicate a Medication Action Plan for the patient.
- Maintain proper documentation of medication-related activities in a software application (i.e. MedPlan).
- Provide education and training on the appropriate use of medications and monitoring devices and the importance of medication adherence and understanding treatment goals.
- Provide proper follow-up and monitoring with patients and prescribers/healthcare providers.
- Responsible for interpretation of physician medication orders for accuracy and appropriateness informing them of any errors omissions or discrepancies and based on an assessment of the patients specific medical needs making real time interventions with the medical provider and staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services.
- Responsible for maintaining accurate and proper patient medication profiles monitoring to ensure the proper choice of drug dosage form strength length of therapy and interaction potential of the prescribed drug.
- Knowledge of practice guidelines or practice parameters; Pharmacoeconomics; quality improvement processes and techniques.
- Critically evaluate a patients medication profile to ensure that each medication is indicated effective safe and that the patient is compliant with therapy.
- Provide drug information to patients nurses physicians or other health care practitioners.
- Be able to critically evaluate and interpret medical information when providing specific information to physicians or providers.
- Performs all responsibilities/duties required by the pharmacy as defined by the scope of service to ensure that the unique nature of the client is addressed.
- Must have excellent written and oral communication skills.
- Is familiar with a variety of pharmacy concepts practices and procedures.
- Relies on experience and judgment to plan and accomplish goals.
- Provides clinical assistance in review/preparation of prior authorization and/or appeals as needed.
- Maintain harmony and a teamwork attitude within the department
- Exhibits an upbeat attitude and a genuine interest in others
- Have a general understanding of and perform in compliance with the current Specialty Pharmacy URAC standards applicable to the position
