Now Hiring: RN Case Manager for Our Assisted Living Facility!
Location: The Pavilion at Robinson Terrace Stamford NY
Schedule: Monday to Friday 8:00am to 4:00pm
Are you a compassionate Registered Nurse with strong clinical skills and a heart for seniors The Pavilion at Robinson Terrace a trusted assisted living community in the beautiful Catskill Mountains is seeking a dedicated RN Case Manager to coordinate care support residents and families and ensure exceptional health outcomes in a warm person-centered environment.
Key Responsibilities:
- Oversee and coordinate clinical care for residents in alignment with individual care plans
- Conduct comprehensive assessments and participate in care plan development and reviews
- Monitor resident health status manage medications and ensure proper documentation
- Communicate regularly with families physicians and interdisciplinary team members
- Provide training and guidance to care staff on clinical procedures and best practices
- Support regulatory compliance and quality assurance initiatives
- Assist in transitions of care and discharge planning as needed
Requirements:
- Active New York State RN license
- Minimum of 2 years of experience in a senior living long-term care or related setting
- Strong clinical assessment critical thinking and communication skills
- Familiarity with NYS Adult Care Facility regulations preferred
- Experience with dementia care and care coordination a plus
