Job Summary:
The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time TH Staff Assistant 2 / Patient Advocate Mother Cabrini Health Foundation. The successful candidate will:
- Be responsible for screening patients.
Conduct social determinant of Health(SOOH) screenings for inpatients and supporting the patients
Help patients through the screening and follow-up of a medical/psychosocial conditions.
Help patients set up appointments and provide financial legal and social support/referrals and communicating with healthcare providers.
Assist patients communicate with their healthcare providers so they can get the information they need to make decisions about their care.
Identify barriers to care such as logistics transportation or language barriers etc.
Gather and document patient issues barriers or concerns. May also need to provide reports for management review.
Required Qualifications:
- High School Diploma.
- Excellent communication and interpersonal skills.
- Empathetic attitude with an ability to offeremotional support.
- Passion for the proper care and well-being of patients.
Preferred Qualifications:
- Bachelors Degree in a healthcare-related field preferred.
- 2 years experience in a healthcare setting preferred.
- Knowledge ofhealthcare systems insurance plans and patient resources.
Work Schedule:
Monday to Friday; 9:00am to 5:00pm (Full-Time)
