Responsibilities

Under the direction of the Executive Director Pharmacy or designee establish and maintain liaison relationships with the medical and office staff in a SPN primary care practice and other settings. Ensure the safe and effective use of medications by assessing for optimal drug product selection dosage regimen design effective administration and drug therapy monitoring services. Provide Medication Reconciliation and Medication Therapy Management tasks as needed documenting on appropriate media and seeking reimbursement where possible. Monitor medication therapies and work with practice staff as appropriate to identify alternatives ensuring provision of best possible evidence-based care. Provide education to staff and support to pharmacy program management and assist in the development of best practices and maintenance of cost-effective pharmacy programs. Provide one-on-one consultations both in an office setting and at patient homes if indicated.

Qualifications

  • Bachelors degree in Pharmacy or PharmD degree required.
  • Valid Pharmacist license in the Commonwealth of Massachusetts or obtainable before initiation of employment.
  • Specific knowledge and training in prescription drugs pharmacy benefits utilization review cost containment strategies and emerging technology.
  • Requires a minimum of 2 years experience functioning as a clinical pharmacist in the area of practice or completion of a pharmacy residency focused in the area of practice as well a working knowledge of all basic aspects of health system pharmacy.
  • Effective oral and written communication skills.
  • Skilled in patient engagement strategies and techniques.
  • Highly self-motivated. Preferred Education Additional Qualifications and Experience: Certification in MTM required in 6 months.
  • Certification to administer immunizations (immunization certification) must be obtained within 6 months.
  • Disease state management or population based certification Diabetes Cardiovascular Respiratory Geriatrics Psych Pain Anticoagulation.
  • Experience working in care teams.
  • Ambulatory care experience long term care experience or other clinical experience.
  • Massachusetts Board of Pharmacy Pharmacist license is required.
  • Valid Pharmacist license in the state of Rhode Island required within 90-days of hire as this position will be focused on supporting our Rhode Island Primary Care locations.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Compensation: Pay rate will be determined based on level of experience.

Pay Range

USD $120556.80 - USD $151278.40 /Yr.
