Central Ohio Primary Care is seeking a full time Advanced Practice Provider (APP) for our COPC Sports Spine & Joint office in Westerville OH. The APP in collaboration with the Physicians coordinates the healthcare of patients in the practice during office visits. This position monitors the patients status and collects data to evaluate and improve care by providing a comprehensive assessment implementing the appropriate treatment and evaluating the patients progress. This is a full time position working Monday-Friday 8:00-4:30 pm. This is a salaried position with opportunities for bonuses based on productivity.
Essential Functions and Responsibilities:
- Develop patient care treatment plans by obtaining medical histories and conducting physical exams of patients during the appointment. This includes establishing a diagnosis and formation of treatment plans. These will be follow-up appointments with patients who have already seen the Physician.
- Patients are seen for spine and musculoskeletal conditions.
- Educate patients and caregivers on conditions outcomes or treatment plan.
- Consult with Physicians and clinical staff regarding issues treatment plan and progress.
- Perform and document peripheral joint injections and trigger point injections in the clinic setting.
- Prescribe medication dosages routes and frequencies based on individual patient needs.
- Order perform and/or interpret results of diagnostic tests when necessary.
- Participate in monitoring patient outcomes. Assure quality of care by adhering to professional standards and procedures set by federal state and company requirements.
- Maintain accurate records of assessments follow-ups and evaluations.
Qualifications
Education Licensures & Certifications:
- Masters Level Degree required.
- Must be considered a Certified Nurse Practitioner (CNP) or Physician Assistant (PA) by nationally recognized board in state of Ohio.
- Experience working in a primary care setting for a minimum of 1 year post graduation as an Certified Nurse Practitioner (CNP) or Physicians Assistant in a Orthopedic Sports Medicine or PMR capacity is preferred.
- Experience with Pain management is preferred.

- EPIC experience is needed. Training is available at onboarding.
