Social Worker Admission Liaison
Mass General Brigham Hospital (Boston)

At Care Dimensions we believe that every moment matters. As a Social Worker Admission Liaison youll play a vital role in ensuring that patients and their loved ones experience comfort dignity and compassionate support during one of lifes most important in collaboration with our interdisciplinary hospice team youll help patients and families understand and embrace the benefits of hospice care. Youll be the guiding presence that eases uncertainty provides clarity and facilitates the transition into the care they needwith empathy professionalism and heart.
Reporting to the Director of Admissions youll:
Join a collaborative team of Care Dimensions clinicians based at MGB Hospital (Boston) to coordinate the hospice admission process to ensure continuity of care.
Serve as a trusted resource to patients families and healthcare providerseducating them about hospice philosophy and services.
Foster strong collaborative relationships with hospitals skilled nursing facilities and community partners to help identify individuals who may benefit from hospice.
Act as a bridge between referral sources and the hospice care team ensuring that each patients journey begins with thoughtful compassionate planning and communication.
Masters degree in Social Work (MSW) from an accredited program.
Current Massachusetts licensure as an LCSW or LICSW in good standing.
At least three years of social work experience ideally including one year in a healthcare or acute care setting.
A deep understanding of hospice philosophy and a genuine desire to serve patients and families at end of life.
Strong assessment skills emotional resilience and the ability to support people through complex and sensitive situations.
Excellent communication collaboration and relationship-building abilities.
Self-directed adaptable and comfortable working both independently and as part of an interdisciplinary team.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following: education training relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Care Dimensions also offers a comprehensive benefits package including employer-sponsored health insurance paid time-off tuition reimbursement student loan paydown supplemental insurance options and more which adds to the companys generous total rewards package.
