Social Worker
Corryong Health (CH) is a public sector agency and one of six multi-purpose services in Victoria. Corryong Health is funded by Commonwealth and Victorian State Government funding and provides a broad range of hospital residential aged care primary health and community support services.
Position Objective/About the Role:
The Social Worker is responsible for providing social work activities in response to referrals from the Corryong Health Multidisciplinary team. The person will be required to have a strong interest in working with older people children with special needs and their families. The SW must be motivated to take initiative in the implementation of relevant projects to meet the needs of our community and ensuring that Corryong Health services are positively promoted in the community.
Position Specific Requirements:
Provide social work support for individuals and families within a multidisciplinary setting
Work across the community acute and residential aged care settings
Work closely with all members of Corryong Health Team to ensure holistic care
Provide a service for families and carers that encompasses a range of flexible and individual responses and support groups
Support clients experiencing Family violence and connect them with specialist services
Provide case management to relevant clients
Participate in the multidisciplinary team programs including continuum of care and carry out case management as required
Undertake individual client assessments using the agency tools
Collect relevant data for Corryong Health requirements monthly
Maintain appropriate client records including contributing to multidisciplinary care plans
Keep Allied Health Manager informed of issues and plans through regular meetings
Regular supervision with an external supervisor (supported with contractual arrangement between Corryong Health and supervisor)
Work with clients for ongoing requirements
Selection criteria
Essential:
of Social Work or relevant tertiary qualification
with the Australian Association of Certified Social Workers
post graduate experience in a relevant area of Social Work
Desirable
3 years of experience working as a social worker
