Medication Services Coordinator (Respite)

Job Overview

Location/Schedule: This position is based out of Rockwood Respite facility located in Gresham OR. The schedule for this position is Monday through Thursday 9:00 a.m. to 5:00 p.m.

Position Description: The Medication Services Coordinator is responsible for overseeing medication management and related services within the facility. This role includes administering medication coordinating with primary care providers (PCPs) and other healthcare professionals ensuring quality assurance and compliance and developing and implementing treatment plans for individuals with severe and persistent mental illness. The Medication Services Coordinator plays a crucial role in program development focusing on the administration and coordination of care to ensure the delivery of high-quality services to those in need.

This position must understand appreciate and respect the diversity and cultural differences within our Cascadia community. Essential Responsibilities:

This position description is not intended to be an all-inclusive list of responsibilities skills or working conditions associated with the position. Management reserves the right to modify add or remove duties as necessary.

  • Manage the administration documentation and evaluation of the medication management program. Train coach and consult with all Residential Counselors who dispense medication.
  • Oversee medication administration room including inventory supply orders tools and materials.
  • Conduct periodic audits of medication room services to ensure quality of service and documentation compliance with all applicable internal and external regulations. Identify and recommend program improvements as necessary.
  • Maintain timely and effective coordination and communication with site medical staff (LMP and nurse); schedule resident appointments follow through on medication orders coordinate client consultations and disseminate key treatment issues to the treatment team.
  • Provide leadership and primary oversight for MAR completion Cycle Fill documentation/ordering pharmacy coordination and communication as well as narcotic medication handling procedures.
  • Schedule medical services appointments with primary care providers specialists dental services and hospitals/EDs.
  • Coordinate with LMP and clinical leadership for effective provision of self-administered medication education and skills building curriculum for all residents including provision of direct training to residents.
  • Facilitate program goals and development while meeting productivity and program expectations and operating within the program budget.
  • Collaborate with program leadership and applicable community partners to develop and implement the regulations policies and procedures under which the program operates.
  • Maintain knowledge of ensure compliance with and review all applicable federal state county and city regulations and approved internal policies and procedures.
  • Document all services in a professional manner using the agency EMR system.
  • Carry partial responsibility for developing and implementing assigned operational functions within the medication room.
  • Provide positive role modeling and mentoring to peers while fostering a client-centered approach by developing maintaining and advancing the consistent use of clinical best practices.
  • Participate in program development and trainings.
  • Provide input on programmatic documentation/ procedural requirements.
  • Provide oversight and continuity of care as well as communication and task assignment during all staff/team meetings shift changes and related procedures.
  • Provide input toward the therapeutic treatment plan in collaboration with the case manager.
  • Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements.
  • Assist in crisis situations as appropriate following emergency protocols/ procedures and coordinating with the treatment team with interventions which support a trauma-informed environment for clients/residents and staff.
  • Demonstrate responsibility for safety of consumers and/or residents staff and property; is familiar with fire regulations and evacuation procedures.
  • Coordinates with oncoming shift regarding resident concerns building/program needs and any issues relevant to ensure the smooth operation of the program.
  • Adhere to mandatory abuse reporting laws and HIPAA requirements.
  • Remain awake on all shifts.
  • Perform other duties as assigned.

Qualifications

Education:

  • Required: QMHA: Bachelors degree in a behavioral science field from an accredited college or university or three years of full-time professional experience providing direct service to individuals in a social service setting. Demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA) as outlined in the Oregon Administrative Rulings (OARs). Certification must be kept current and in compliance with OARs through MHACBO.

Experience:

  • Experience working with adults with severe and persistent mental illness.
  • Training and/or demonstrated proficiency in medication administration is preferred.

Specialized Knowledge Skills and Abilities:

  • Understanding of components of maintaining a trauma-informed care environment
  • Operate computer hardware and software at a level needed to effectively perform job functions
  • Ability to interpret follow and communicate complex instructions. Incumbent must be detail-oriented and attentive to accuracy.
  • Ability to enter and retrieve data
  • Ability to prepare simple reports

Other:

  • Possess a valid drivers license three year driving history; clean driving record ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
  • Ability to perform CPR First Aid and properly use AED equipment. CPR/ First Aid certifications are required and must remain current.

Working Conditions

Work is performed in a residential supportive housing setting. Daily routines can be hectic and will likely require the Medication Services Coordinator to perform various functions of the site and on occasion perform the tasks of other employees as needed. Meal periods and breaks are subject to interruption. There is a moderate to high risk of personal injury/illness due to clients with violent behavior exposure to disease biohazards noise and contaminants. Universal Precautions and remaining alert to the environment are critical to address the potential for these hazards.

Mental Demands: The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach.

Physical Demands: The Medication Services Coordinator must have the ability to assist residents in participating in a variety of active leisure and recreational events as well as skill building outings and accessing community resources. The Medication Services Coordinator must have the ability to assist the team in providing safety to all residents and staff in potentially volatile situations.
